Description:

LPN ratios consist of 2 LPNs per shift. 1 LPN is designated for memory care (15 beds) and 1 for skilled care (15 beds). They will split responsibilities for assisted living patients (20 beds), up to a maximum of 25 patients. A goal of having 3 LPNs per shift is aimed for during the day. Common patient diagnoses include CHF, HTN, diabetes, strokes, Alzheimer's, and dementia.

Responsibilities will include administering tube feedings, blood sugars, insulin, vaccinations, and medications (PO, SQ, IM). Skin assessments will also be part of the duties. The director will handle admissions, and no discharges are performed. Charting is by exception. There is a presence of nurse aides on the unit. Equipment used includes O2 and CPAP. There are no trachs or chronic vents on-site.

Breathing treatments and inhalers may be utilized. Shifts consist of 8-hour rotations. A rotating schedule is implemented, accommodating requests for PTO or FMLA. Weekend shifts are required every other week. If needed, holiday shifts may also be required. LPNs will be trained to float to all units within the facility. Dress code permits any solid color scrubs. Facial piercings and offensive tattoos are not allowed.

Smoking (tobacco/marijuana) is prohibited on premises, including designated areas. No earbuds or personal phones are to be used in patient care areas. Breaks are provided during shifts.
