Description:
Ratios are 2 LPNs; 1 on memory care (15 beds) and 1 on skilled (15 beds), splitting assisted living (20 beds) with a maximum of 25 residents. For day shifts, the goal is to have 3 LPNs per shift. Required certifications include BLS, with IV certification as a bonus. Common diagnoses and types of patients include those with CHF, HTN, DM, history of strokes, Alzheimer's, Dementia, and total cares.
Job duties include administering tube feedings, blood sugar checks, insulin, vaccinations, and PO, SQ, and IM medications, as well as performing skin assessments. The Director will handle admissions, while discharges are not applicable as residents stay for life. Charting is done by exception, and care spans all levels:
Independent living, Skilled Nursing Facility, and Memory care. Experience or required skills include a minimum of 1 year as an LPN, with hands-on care and assistance to CNAs being essential. There are nurse aides present on the unit. The nursing staff will have access to O2 and CPAP equipment, but there are no tracheostomies or chronic vents. Shifts consist of 8-hour schedules, including 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, and 10:00 PM - 6:30 AM, often requiring flexibility to accommodate scheduling needs.
Weekend requirements are every other weekend, with holiday coverage as necessary. Floating across units may be required, and training will be provided. Scrubs should be in solid colors. Additional notes include restrictions on facial piercings and tattoos, as well as a no-smoking policy on the premises. Earbuds and phones are not permitted in patient care areas, and breaks are limited to designated times.
