DescriptionOf Work
This role focuses on connecting older adults with services, resources, and supports that meet their needs. It involves gathering information, determining eligibility, and guiding consumers through various programs. As an Aging Case Aide 2, you will perform the following duties:
- Referral Processing: Review and record incoming referrals received through telephone calls, mail, faxes, police reports, and other sources
- Microsoft Software Skills: Create fliers, calendars, spreadsheets, and other materials for advertising and organizational purposes
- Information Screening: Screen calls, identify presenting problems, provide information, and collect detailed referral data
- Eligibility Assistance: Determine benefits eligibility and assist consumers with completing applications for programs such as prescription coverage, tax rebates, housing, and transportation
- Case Documentation: Document all consumer contacts with thorough and accurate case histories, resolutions, and dispositions
- Community Support: Participate in wellness fairs, coordinate holiday gift events, and offer guidance on agency services and community resources
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

Qualifications
Minimum Experience and Training Requirements:
- One year as an Aging Case Aide 1; or
- Two years of public contact work, including one year in a human services program; or
- Any equivalent combination of experience and training.
Other Requirements:

- You must be able to perform essential job functions.
