- WBASD Disclosure Statement
- Act 168, Sexual Misconduct/Abuse Disclosure Release
- Arrest/Conviction Report & Certification
- All Clearances required by PDE (State Police Criminal Background, Child Abuse Background & FBI Fingerprinting)
- Mandated & Permissive Reporter Training
- Physical and TB test
