Position: CNA / CHHA (Berks County, PA) New Berks County, PA

Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, and Georgia. Ennoble Care’s clinicians go to the home of the patient, providing continuum of care for those with chronic conditions and limited mobility. Ennoble Care offers a variety of programs including remote patient monitoring, behavioral health management, and chronic care management, to ensure that our patients receive the highest quality of care by a team they know and trust.

Responsibilities
  • Care for a census of 5-6 patients.
  • Provide or assist in ADL activities: personal hygiene, grooming, mouth, skin care, and nails.
  • Assist with toileting, positioning, transfer activities, frequent turning, and positioning of a bed‑bound resident or patient.
  • Assist with prescribed exercise and ROM activities taught by the Hospice Nurse.
  • Assist with light housekeeping of individual’s unit or room, including changing bed linens and patient laundry.
  • Feed and assist with meals.
  • Report any changes in resident’s or patient’s condition to the Nurse.
Qualifications
  • Valid Pennsylvania CNA or CHHA certification.
  • Valid driver’s license and reliable mode of transportation.
  • Authorized to work in the United States.
  • Fully vaccinated against COVID‑19.
