UPMC Community Medicine Incorporated is hiring a Full-Time Certified Registered Nurse Practitioner to help support UPMC Partners in Health.
Hours: Monday-Friday daylight
Location: Murrysville and Delmont
Position Specifics: Must demonstrate strong clinical assessment, diagnostic, and treatment planning skills, including chronic disease management, preventive care, and care of complex patients across the lifespan in ambulatory settings. Requires the ability to thrive in a fast-paced, team-based environment with a commitment to evidence-based, high-quality outcomes, strong communication and patient engagement skills, and proficiency in EHR documentation, patient messaging, inbox management, and value-based care initiatives.
Department Details: A large, well-established family medicine practice serving patients of all ages across four locations in Westmoreland County. Our team includes 19 physicians and 6 APPs. APPs carry their own established patient panels and also provide care for patients of other providers within the practice to support timely access and comprehensive coverage across the care team. We operate within a strong, collaborative APP model led by an APP supervisor.

Staff/Senior Level: The Staff or Senior Advanced Practice Provider (APP) is a Physician Assistant or Certified Registered Nurse Practitioner and a member of the care delivery team. The APP is responsible to set the standards for the level and quality of care. The APP has responsibility, authority, and accountability for the provision of care through supervision by and collaboration with a physician.
The APP manages and provides patient care activities for a group of patients and their families through the application of independent judgment, communication, and collaboration with all team members. The role of the APP encompasses the beginnings of leadership, partnership, collaboration, and supervision. The APP establishes and maintains collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care.
The APP begins to demonstrate a commitment to the community and to their healthcare profession.
The successful candidate will be placed according to years and months of experience for employment purposes.
