In-Home Traveler, Nurse Practitioner or Physician Assistant - PA

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you’ll travel up to 100% of the time across a defined coverage area, with overnight hotel stays, car rentals, or flights as necessary.

Holding multiple state licenses will be required, as dictated by business needs.

Visit lasts about 45 minutes and allows time to connect one-on-one and answer health questions. It includes a medication and medical history review, a physical evaluation, and optional diagnostic tests such as diabetic eye exam and spirometry. You’ll use an iPad and an intuitive clinical workflow for each evaluation. Physicians are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, DEE, and bone density screening.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
• Able to cover all counties within state and flex to other areas of business need.

PA Coverage Markets Include:

$87,035.00 – $ (typical for this role). The actual base salary will depend on experience, education, geography and other factors.
