As a Licensed Practical Nurse Weekend Baylor (LPN), you will be responsible for providing direct patient care to our residents, including administering medications, performing vital signs, and assisting with activities of daily living. You will also maintain accurate and detailed records, collaborate with our interdisciplinary team, and promote a positive and respectful environment.

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living

• Maintain accurate and detailed records, including medication records, treatment plans, and daily progress notes

• Collaborate with our interdisciplinary team to develop and implement individualized care plans

• Assist with resident admissions, transfers, and discharges

• Participate in quality improvement initiatives and performance improvement activities

• Promote a positive and respectful environment, ensuring excellent customer service to residents, families, and visitors

• Current LPN licensure in the state of Pennsylvania

• High school diploma or equivalent required

• Previous experience in a long-term care or rehabilitation setting preferred
