• Provide competent nursing care by executing job responsibilities safely and consistently.
• Respond to incoming telephonic requests in a dependable manner.
• Provide clinically competent triage and symptom management to patients.
• Utilize standardized protocols for medication management and prescription refills.
• Conduct thorough and accurate reviews of patient medications.
• Provide comprehensive education and direct patient care, particularly around chronic conditions.
• Actively collaborate and monitor the implementation of care plans for patients.
• Form relationships with patients and caregivers to support preventative care.
• Create a welcoming environment to meet the needs of patients and communities.
• Participate in care team meetings to discuss patient care and clinic operations.

• Education:
  • Active Registered Nurse (RN) Licensure in good standing with the applicable state
• Experience:
  • 2+ years of healthcare experience, working as an RN preferred
