Aging Care Manager 2 (Local Government) - Lebanon County Area Agency on Aging

• OPTIONS Caseload:
  Visit consumers in their homes for intake, general casework, and assessments that involve interviewing, evaluating needs, and developing a plan of care for services
• Service Coordination:
  Arrange and monitor services for consumers with simple to complex needs, help consumers complete applications and utilize resources
• Assessment Work:
  Conduct initial, six-month, and annual reassessments by phone and in person at homes, facilities, and hospitals
• Record Management:
  Document case history, care plans, and ongoing activity; complete reports; and maintain accurate files
• Community
  
  Collaboration:
  
  Work with families, providers, and community agencies to support consumer needs
• Teamwork Support:
  Serve as backup for the Person-Centered Care Manager and receive one PCC referral at a time

• Full-time employment
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Telework:
  You will not have the option to telework in this position.

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.
