Home Health Visit Nurse (RN only)
Active Ohio RN in good standing is required.
At least 1 year of paid experience at a home health agency (or equivalent) is required; 2‑5 years’ experience is preferred.
1+ years of experience in Case Management, wound care, PICC/PORT/IV Care, lab draws, OASIS and Non‑OASIS assessments is highly desirable;
Nurse Supervision experience is a plus.
- Required schedule availability:
Monday – Friday (weekdays) from 8 am – 5/6 pm. - Part‑time visit nurse requires a minimum travel radius of Stark, Mahoning and Trumbull Counties plus surrounding areas up to 60 min (up to 90 min on occasion).
- Full‑time visit nurse region includes Mahoning, Trumbull, Stark, Ashtabula, Columbiana, Portage and Carroll counties and areas up to a 2‑hour travel radius. Additional cases will be added if more hours are desired (not required).
- Qualified applicants may reside in or near any primary service area so long as travel requirements are fulfilled throughout the assigned region.
- No On‑Call requirements / No Overnight shifts required.
- No required weekends (optional when available).
- Overtime, Respite & PRN services are offered when available but are not required.
Starting pay rate: RNs $35 – $40 per hour.
- Weekly pay (Fridays) including onboarding, training and an additional 4 hours on the first paycheck.

- Graduate of an accredited school of professional nursing.
- Active RN, with current license to practice as a Registered Nurse in the state of Ohio.
- At least 1 year in home health industry + 1 year of nursing experience; additional home health nursing experience is highly desired.
- Active CPR and First Aid certificates highly desired.
- Minimum of 1 year direct patient experience with vent care.
- Good interpersonal communication skills – written and verbal.
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly and accommodating, respectful.
- Current driver's license or state reliable transportation.
- Excellent attendance record with past employers.
- Ability to meet all basic job requirements.
- Willingness and ability to pass state and federal background check.
- Provides professional nursing care under the direction of a Registered Nurse and communicates status changes to the RN.
- Provides skilled nursing visits for clients as directed by the RN case manager following the care plan.
- Recognizes and reports changes in client condition to the case manager and physician.
- Communicates information in a timely manner and documents accurately in the clinical record.
- Scope of clinical practice includes personal care services, assessment, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope and as delegated.
- Individuals with the license designation “LPN M‑IV” may perform venipuncture for lab samples.
- Promotes personal safety and a safe environment for clients and coworkers.
- Demonstrates knowledge of safety/infection control practices and compliance with policies.
- Recognizes and responds appropriately to unsafe situations.
- Demonstrates safe practice in use of equipment.
- Makes recommendations for safety improvements.
- Performs job in compliance with agency policies and professional community standards.
- Accepts responsibility for professional nursing role.
- Follows applicable state and federal laws and agency standards.
- Attends required meetings.
- Maintains confidentiality of all aspects of the job.
- Refrains from disclosing client or staff identities or circumstances in public.
- Participates in development, implementation, and evaluation of the Agency Quality Improvement Program.
- Performs other related duties as deemed necessary.
- Frequent standing, walking, bending, stooping; occasional sitting.
- Weight handled; push/pull force 10‑20 lbs occasionally, 20‑30 lbs infrequently; carry/lift 5‑10 lbs occasionally, 10‑30 lbs infrequently.
