Job Title
Home Health Visit Nurse (RN only)
Qualifications- Active Ohio RN in good standing.
- At least 1 year of paid experience at a home health agency (or equivalent) is required; 2‑5 years preferred.
- 1+ years of experience in case management, wound care, PICC/PORT/IV care, lab draws, OASIS and non‑OASIS assessments. Nurse supervision experience is a plus.
- Graduate of an accredited school of professional nursing.
- Active CPR and First Aid certificates highly desired.
- Minimum of 1 year direct patient experience with ventilator care.
- Good interpersonal communication skills, written and verbal.
- Must hold a current driver’s license or state have reliable transportation.
- Excellent attendance record.
- Willingness and ability to pass state and federal background check.
- Must be available Monday–Friday weekdays 8 a.m.–5/6 p.m.
- No mandatory on‑call, overnight, required weekend obligations; optional weekend work may be offered.
- Part‑time or PRN visit nurses must be within a 60‑minute travel radius of Stark, Mahoning, and Trumbull counties (up to 90 minutes on occasion).
- Qualified applicants may reside in or near any primary service area so long as travel requirements are fulfilled.
- Long‑term employment for part‑time or PRN positions.
- Opportunity to expand into larger part‑time or full‑time roles.
- Mandatory weekly schedule; not mandatory short‑term or mandatory PRN/on‑call roles.
- RNs: $35 – $40 per hour, based on qualifications, certifications, experience, skill specializations, and client & team fit.
- Autonomy in a client‑centered culture with quality and compliance support and training.
- Weekly pay (Fridays) including onboarding, training, and an additional 4 hours paid on first paycheck.

A successful Visit Nurse at HCN performs skilled nursing functions in compliance with federal, state, and local laws and the Nurse Practice Act, under the direction of a RN Case Manager.
Essential Functions- Provides professional nursing care under RN direction and communicates status changes per agency guidelines.
- Conducts skilled nursing visits as directed by the RN Case Manager following the care plan.
- Recognizes and reports changes in client condition to the case manager and physician.
- Clinically documents information promptly and accurately.
- Performs personal care services, assessment, medication monitoring and administration, client teaching, emergency intervention, and services within professional scope.
- May perform venipuncture if licensed as LPN M‑IV.
- Promotes client and coworker safety and complies with infection control policies.
- Responds appropriately to unsafe situations and recommends safety improvements.
- Accepts responsibility for professional nursing role, follows state and federal laws, attends meetings, maintains confidentiality, and participates in quality improvement activities.
- Performs other related duties as required.
- Frequent standing, walking, bending, stooping; occasional sitting.
- Weight handling: push/pull 10‑20 lb occasionally, 20‑30 lb infrequently; carry/lift 5‑10 lb occasionally, 10‑30 lb infrequently.
