Home Health RN

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Alpena, Michigan. The position supports patients in Alpena and surrounding areas.

Responsibilities
  • Lead and collaborate to provide management and delivery of patient care plans as a home health registered nurse case manager.
  • Provide clinical care and support to patients throughout the region.
Qualifications
  • RN license in the state of employment.
  • Graduate from an approved school of practical nursing.
  • One year of RN experience and clinical competence to deliver quality patient care.
  • Current driver’s license; ability to drive ~20% of your day to patient locations.
  • Commitment to consistently meet critical deadlines for charting.
  • Ability to self‑manage time and schedule.
  • Demonstrated experience with tablets, mobile phones and EMR software.
