Per Diem LPN – Home Health – Atlanta

Pay: $50 per visit.

Accord Care is seeking a Per Diem Licensed Practical Nurse (LPN) who is committed to delivering compassionate, skilled care in the home setting. This role offers the opportunity to provide one‑on‑one support that promotes client health, independence, and dignity.

As a vital member of our home health team, you will:

• Provide skilled nursing care tailored to each client’s unique needs
• Monitor health conditions and document changes in clinical status
• Administer medications and perform wound care
• Deliver G‑tube, trach, or vent support when required
• Educate and guide clients and families to support ongoing health at home
• Communicate effectively with the care team to ensure continuity of care

• Licensed as an LPN and in good standing with the Georgia Board of Nursing
• At least one year of clinical experience preferred
• Certified in CPR
• Skilled in EMR documentation with strong organizational ability
• Confident working independently in a home environment
• Compassionate, client‑focused, and a strong communicator
• Experience with G‑tube, trach, or vent care
• Licensed to drive with reliable transportation
