About This Role

The Telephonic RN Nurse Case Manager I - Ameri Ben is responsible for performing care management for members with complex and chronic care needs. This role is ideal for experienced nurses who can assess, develop, implement, coordinate, monitor, and evaluate care plans to optimize member health care.

  Full-time, Monday thru Friday 8 am to 5 pm

• Perform care management within the scope of licensure for members with complex and chronic care needs.
• Conduct assessments to identify individual needs and develop specific care management plans.
• Implement care plans through actions based on assessments, including providing education and facilitating authorizations/referrals.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate the effectiveness of care management plans and modify as necessary.
• Interface with Medical Directors and other health professionals on care management treatment plans.
• Assist in problem-solving for members and providers related to access to care, vendors, claims, or service issues.

Education:

• BA/BS in a health-related field required

Experience:

• Minimum of 3 years of clinical experience required
