Overview Hospice Clinical Liaison - RN

M-F / limited on-call
The Clinical Liaison, either a Registered Nurse (RN), or Social Worker, will act as liaison between the contracted hospitals and the hospice team, make inpatient visits to hospice patients in contracted facilities, act as consultant for pain and symptom management, provide in-service education to providers, perform pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient/family of hospice and its services.
Also obtain informed consents. Provide assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life and coordinates discharge planning.
If you meet these qualifications, we would love to meet you:
- One (1) year of RN experience in local market with a strong knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques
- Previous medical sales experience preferred
- Licensed RN currently licensed as a registered nurse in the state of agency operation
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
- Provide community and provider in-service education in assigned territory.
- Maintain daily communication with Admissions Department and Team Directors.
- Work with the Hospice Care Consultants to develop and implement individualized marketing plan.
- Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
- Act as a liaison with Accent Care Hospice Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families.
- Make inpatient visits at contracted facilities and communicates with Admissions Department, Team Directors and/or Primary Nurse.
