Summary of Position:
Provides support services, information, education and support for clients. Promotes wellness, recovery and skill building to sustain better health. Provides prevention training for managing stress and problems, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery. Assists clients with personal recovery goals, assisting them in developing a natural support systems, and coaches clients to advocate for themselves within their healthcare systems.
Essential Duties & Responsibilities:
- Establishes relationships and practices core coaching skills to transform client values/desires into action and maintain lasting change over time.
- Provides group and/or individual training/interventions focusing on improvement of the client's functioning and health-related issues.
- Provides wellness consultation. Assists clients with developing their Wellness Recovery Action Plan (WRAP).
- Empowers clients to self-manage chronic health conditions and solidify wellness goals.
- Links clients to community-based social support/services.
- Provides ongoing case management services for assigned service recipients.
- Provides transportation assistance to help clients achieve and maintain wellness goals and initiatives.
- Conducts intake for recipient entering services.
- Assists service recipient in making discharge plans and provide follow up services.
Knowledge, Skills & Abilities:
- Effectively communicate via written, verbal, in person and virtual methods.
- Skill in customer service/experience techniques.
- Skill in problem-solving.
Qualifications:
Education Level: Associate's degree and 1 year of participation in own personal recovery process in order to care for self. Participation in personal recovery process or work experience equivalent to 3 years required. Must have the ability to share their recovery journey.
Experience:
Experience providing mental health services preferred.
Certification/Licensure:
Ability and willingness to obtain credentialing if required.

Pay Range: $15.75--$22.00
