CCBHC Community Outreach Counselor

Overview

• Develop and implement outreach strategies to increase awareness of services among community members
• Build and maintain relationships with community organizations to enhance client referrals
• Conduct enrollment and assessments for eligible participants while maintaining accurate documentation
• Evaluate community needs and adapt outreach strategies to better serve the community
• Coordinate outreach activities and ensure continuity of care in collaboration with team members and community partners

• Master’s Degree in a Human Services Field, with licensure eligibility required
• Minimum of one year of experience providing behavioral health services
• Knowledge of community engagement strategies and relationship-building techniques
• Proficiency in Microsoft Office Suite and Electronic Health Records
• Strong customer service skills and ability to prioritize tasks effectively

• One year of experience in community outreach or a similar field
