Position: Registered Nurse (RN) - Home Health - $75K-100K per year

Position

Registered Nurse (RN) – Home Health – Full Time, 8‑hour shifts, 40 hours per week, located in Alton, Illinois.

Overview

Residential Home Health and Hospice is seeking a dedicated RN to serve as a home health case manager. The role leads and collaborates with the interdisciplinary team to manage and deliver patient care plans across Alton and surrounding areas.

Responsibilities
  • Develop and manage individualized patient care plans.
  • Provide nursing care and conduct assessments at patients’ homes.
  • Document care and progress in the EMR accurately and within critical deadlines.
  • Coordinate with allied health professionals and family members to ensure quality care.
  • Assist with scheduling, travel itineraries, and logistics for patient visits.
  • Participate in clinical outcome reviews and use performance data to improve care delivery.
Qualifications
  • Active RN license in the state of employment.
  • Graduate from an approved school of practical nursing.
  • Minimum one year of RN experience with demonstrated clinical competence.
  • Valid driver’s license and ability to travel approximately 20% of the time to patient sites.
  • Proficiency with tablets, mobile devices, and EMR software.
  • Strong time‑management and self‑management skills.
  • Commitment to meet charting, coding, and other regulatory deadlines.
Compensation & Benefits
