SummaryThe Social Services Coordinator is responsible for the evaluation and assessment of resident’s social service needs and concerns. Develops relationship with residents and families and ensures that all needs and concerns are met within a timely manner. Initiates Social Service charting and development of an activity program to meet resident's physical and mental stimulation needs upon admission and continues to monitor residents quarterly and completes charting as required.
Demonstrates accountability for assessing, updating, and maintaining resident's rights through observation and assessment. Completes quarterly progress notes, social service log and participates in Resident Care Plan meetings.
- Bachelor's degree in social work required
- Must have knowledge of growth and development and show expertise for geriatric population
- Two years experience preferred
- Geriatric and Social Work experience preferred
- Excellent organizational and communication skills
- Ability to perform necessary documentation in writing and electronically
- Manual dexterity and skillful use of hands when working with patients and equipment
