Role Overview

This position involves assessing residents' health, educational, and environmental needs, developing and updating individualized care plans through interdisciplinary collaboration, and ensuring safe delivery of nursing interventions. The nurse communicates effectively with team members, residents, and families, documents all assessments and interventions accurately, and responds promptly to changes in resident conditions using critical thinking skills. Maintaining professionalism during crises and monitoring care effectiveness are essential.

The role also includes participating in ongoing education and staying current with best nursing practices.

• Accredited nursing education leading to RN licensure and current state licensure
• CPR/AED certification (or willingness to obtain)
• Preferred experience in long-term care
• Strong critical thinking, communication, and organizational skills
• Physical ability to stand, walk, lift, and assist with residents and equipment
• Manual dexterity and visual acuity for documentation and patient care

• Assess residents and document findings
• Create and revise personalized care plans
• Implement nursing interventions safely and effectively
• Communicate with the healthcare team and residents' families
• Respond to resident changes and emergencies professionally
• Participate in ongoing education and quality improvement processes
