Shift ScheduleDays: 6:00am – 2:30pm
Evenings: 2:00pm – 10:30pm
Nights: 10:00pm – 6:30am

We are seeking a dedicated LPN – Licensed Practical Nurse to join our team. The LPN Charge Nurse plans, organizes, and assists in providing direct and indirect resident care following established policies and procedures, standards of nursing practice, and principles of nursing care. They orient, instruct, and supervise personnel.
Responsibilities- Assess and implement interventions for resident care based on standards of nursing practice and principles of nursing care, in accordance with Federal, State, and company requirements.
- Make daily rounds to see all residents to ascertain condition and comfort.
- Initiate procedures to protect residents from injury and infection.
- Assist the DON and/or RN with developing, implementing, and evaluating plans of care to include, but not be limited to, recognition of spiritual, emotional, psychosocial, and functional factors.
- Encourage and assist with restorative care in self‑care and group activities.
- Monitor nursing personnel compliance with nursing policies and procedures and competency of clinical skills, evaluating nursing staff job performance with direction from DON.
- Communicate with the physician as indicated by resident needs or status and assure follow‑through; maintain hand‑off communications with oncoming shift regarding resident care needs.
- Oversee direct care staff and ensure staff complete tasks and care as assigned.
- Speak, read, and write the English language in an understandable manner.
- Apply principles of basic general math.
- Graduate from an accredited school.
- Currently licensed in the applicable state without encumbrance, responsible for maintaining professional licensure and CEU requirements.
- Preferably 1–2 years of previous nursing experience as a Charge Nurse in long‑term care and/or a skilled nursing facility.
- Currently CPR certified.
