Physical Therapist (PT) Home Health Care – Travel assignment in Altoona, Pennsylvania.

Centra Healthcare Allied is seeking a travel Physical Therapist for a 26‑week assignment in Altoona, PA, with 36 hours per week (7.2‑hour days). The role involves assessing referred patients, developing individualized plans, and providing direct patient care under physician direction.

• Assess and evaluate referred patients requiring rehabilitation services.
• Establish and administer treatment programs, setting realistic goals based on the patient’s capacity and tolerance.
• Document and record patient status, progress, and results.
• Educate patients and families in physical therapy methods.
• Collaborate with interdisciplinary team to plan and evaluate patient goals.

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist or actively pursuing licensure.
• Current CPR certification.
• Ability to develop a care plan and provide skilled physical therapy services per physician orders.
• Experience in assessing patient needs, planning, evaluating, and modifying therapy interventions.
