** Position Summary
** The RN Case Manager coordinates and manages nursing care for individuals with Intellectual and Developmental Disabilities (IDD) in home and community settings. This role involves comprehensive case management, including assessing individual needs, developing and implementing care plans, and ensuring seamless coordination across interdisciplinary teams. The RN Case Manager serves as the primary liaison for individuals and their families, guaranteeing that all healthcare services are delivered efficiently, compassionately, and in compliance with ODP (Office of Developmental Programs) and state regulations.

* Key Responsibilities:

**** 1. Care Coordination and Case Management
*** Assess physical, emotional, and developmental needs of individuals with IDD in home settings.
* Develop individualized care plans in collaboration with Supports Coordinators, physicians, therapists, and other healthcare providers.
* Serve as the primary point of contact for individuals and families, ensuring clear communication and service coordination.
* Monitor progress and modify care plans as needed based on evolving needs and assessments.
* Coordinate schedules and staffing to meet appropriate levels of care.
* Conduct in-person 60-day recertifications to reassess care effectiveness and compliance.
** 2. Referrals, Staffing Coordination, and Onboarding
*** Generate referrals from community partners for individuals needing IDD services.
* Collaborate with the RN Nursing Manager and Business Operations Manager to assess staffing requirements and allocate resources effectively.
* Oversee the referral process to ensure timeliness, accuracy, and alignment with individual needs.
* Review resumes, conduct interviews, and evaluate candidates for appropriate placement.
* Coordinate onboarding, ensuring completion of all background checks, documentation, and certifications.
** 3. Collaboration and Communication
*** Maintain ongoing communication with interdisciplinary teams, including physicians, therapists, social workers, and caregivers.
* Educate individuals and families about health conditions, care routines, and home management strategies.
* Address barriers to effective care and ensure family satisfaction through responsive support.
** 4. Compliance and Documentation
*** Oversee and review daily nursing documentation from field-based RNs and LPNs.
* Provide training and guidance on best practices in nursing care, person-centered planning, and compliance with internal policies.
* Maintain accurate and up-to-date records, including ISPs, assessments, nursing notes, and incident reports.
* Ensure timely and accurate reporting of all incidents in accordance with regulatory requirements.
** 5. Health Education and Prevention
*** Educate individuals and families on condition management, medication administration, and preventive health.
* Promote healthy lifestyle practices and strategies for improving quality of life and independence.

* Qualifications:

**** Education
* ** Bachelor’s or Associate Degree in Nursing (BSN or ADN) from an accredited program.
** Licensure:
*** Valid Registered Nurse (RN) license in Pennsylvania.

* Experience:

*** Minimum 2 years of nursing experience, preferably in home health or IDD care.
* Experience in case management, care coordination, or interdisciplinary collaboration preferred.
