As a Hospice RN – Regional Support, you'll provide direct patient care while serving as a floating clinician assigned to hospice branches based on census needs, growth opportunities, or staffing support.
Responsibilities Include- Managing and coordinating hospice patient care
- Performing comprehensive patient assessments
- Developing and implementing individualized care plans
- Educating and supporting patients, families, and caregivers
- Collaborating with interdisciplinary hospice teams
- Providing compassionate care that promotes comfort, dignity, and quality of life

As a Regional Support Hospice RN, You Will- Deliver high‑quality, compassionate nursing care for hospice patients
- Manage and implement individualized plans of care
- Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
- Assess and address patients' physical, emotional, spiritual, and environmental needs
- Accurately document all care provided and ensure compliance with hospice protocols
- Educate families and caregivers on care instructions, medications, and safety
- Participate in the on‑call rotation and provide after‑hours care as needed
- Collaborate with physicians, referral sources, and the care team
- Assist with referrals, regulatory documentation, and payer compliance
- Travel to various Gentiva hospice locations as assigned to fill urgent needs
- Regional travel with overnight stays based on assignment needs
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience (hospice, home health, acute care, long‑term care, oncology or other high acuity setting)
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
- Hospice and end‑of‑life nursing experience
- Skilled in clinical assessments and compassionate communication
- Strong critical thinking, flexibility, and problem‑solving skills
- Comfortable working in varied environments with shifting needs
- Proficient in documentation and electronic health records
- Calm and supportive in crisis situations; skilled at family engagement
