Interim Health Care Hospice & Palliative Care in Altoona, PA is seeking a Hospice Registered Nurse Case Manager (RN) to provide compassionate, patient-centered care. You will oversee and coordinate the patient's plan of care, collaborate with interdisciplinary team, and support end-of-life comfort and dignity.
The ideal candidate holds an ADN/AAS with RN license in Pennsylvania (BSN preferred), CPR/FA certification, and at least one year of RN hospice experience.
