LPN - Home Care

Role:

Licensed Practical Nurse (LPN)

• Provide in-home patient care according to prescribed plans, monitoring progress and recovery
• Assess, document, and report patient conditions, including vital signs and symptoms
• Administer medications, insulin, and IV fluids with thorough documentation
• Inspect and care for wounds, change dressings, and assist with personal hygiene
• Evaluate for injuries, infections, or bedsores and deliver appropriate treatments
• Collaborate with healthcare providers and team members to ensure quality patient outcomes

• Graduate of an accredited nursing program
• Current unrestricted LPN license in the state of practice
• Current CPR, AED, BLS, and First Aid certifications
• Reliable transportation to care sites
• Optional but preferred: practical trach and/or ventilator experience

• Deliver personalized, direct patient care in-home setting
• Monitor and document patient health status and progress
• Administer medications and treatments accurately
• Maintain wound care and hygiene standards
• Collaborate with healthcare team to optimize patient wellbeing
