This position is responsible for assisting expectant, custodial, and non-custodial young parents under 22 years of age in obtaining a high school diploma or its equivalent. The focus is to help students remain in school in order to secure post‑graduation employment, while also provide education and training the will help them become successful parents and self‑sufficient adults.

QUALIFICATIONS EDUCATION/EXPERIENCE AND/OR TRAINING
  • Bachelor’s Degree in Social Work or related field
LICENSES OR CERTIFICATIONS
  • Valid Pennsylvania driver’s license
WORK-RELATED KNOWLEDGE
  • Experience working in social service environment and/or with social service agencies.
  • Must have strong interpersonal skills in order to relate well with entire family unit.
RESPONSIBILITIES
  • Provide intensive case management services to pregnant and parenting students per the funding guidelines.
  • Conduct a detailed intake session with new students and gather all required enrollment documentation.
  • Conduct home visits and make relevant referrals for educational and community resources.
  • Monitor student attendance and academic status and address identified barriers accordingly.
  • Identify community resources and services; make appropriate referrals to support needs.
  • Conduct bi-weekly group sessions and monthly workshops covering required components including year-round flexible services.
  • Assist with transition back to school after baby is delivered; assist with transition to workforce or postsecondary education/training after graduation.
  • Maintain individual case management files, including detailed case notes in accordance with funding guidelines.
  • Serve as an advocate for pregnant and parenting teens, represent the Department and District accordingly at the local and state level.
  • Attend case management professional development courses and required trainings, which may require overnight travel.
