- Evaluates and performs assessments of swallowing, speech, and language to identifytreatment plans and avenues for individuals.
- Proactivelycreates individualized treatment plans and ensure the individuals carries outtheir treatment plans by setting goals and objectives addressing specificfunctional needs for individuals.
- Demonstrateto individuals how to make sounds while improving their voices and maintainingfluency.
- Provides support, counsel, advice, training and education for individuals, family,caregivers and other professionals.
- Efficientlyassembles extensive documentation on individuals’ progress, care services andtherapy adjustments, when necessary, that aligns with standard medical documentation and facility protocol/procedures documentation.
- Collaborativelywork with other healthcare professionals to provide comprehensive care.
- Mustdisplay excellent active listening skills, critical thinking skills,verbal/written communication skills, time management skills.
- Mustdisplay compassion, integrity and respect to all individuals.
- Supportingadditional responsibilities and/or tasks as needed for the department.
- Physically,be able to lift, transfer and assist patients while providing therapy interventions during physical activity.
- Must beable to lift and carry items up to 25lbs, including assisting individuals when needed.
- Abilityto visually be able to perform job duties, including examining individuals an dreading information from printed sources and computer screens.
- Education:
Must be a graduate of an accredited Speech Language Pathologist therapy program - License/
Certifications:
Must hold an active and in good standing Wisconsin Speech Language Pathologist License and CPR Certification. - Follows/adheres to organization, federal, andstate regulations and guidelines, including Resident/Client rights
