Position Purpose

With minimal supervision; assists in performing health screenings on members to evaluate a member’s medical and social needs for potential enrollment into the case management program. Relates effectively with others for continuity of care; maintains satisfactory relations with others, maintains accurate and complete records.

Essential Position Functions
  • Perform health screening assessments on our members both by making outbound calls and taking inbound calls;
  • Helps coordinate care and connect members with resources;
  • Work closely with the Case Management team to ensure members are appropriately enrolled into the case management program;
  • Advocate on behalf of the member regarding accessibility of services, assessing and interpreting member needs;
  • Identifies problems and assists with the best possible solution in an accurate and prompt manner;
  • Solves moderately complex problems and/or conducts moderately complex analysis;
  • Communicates in a pleasant, professional, and efficient manner assuring exceptional customer service
  • Prioritizes and organizes own work to meet agreed upon deadlines;
  • Relates effectively with co-workers regarding continuity of care and Care Management of members;
  • Maintains a professional approach with the highest standard of confidentiality;
  • Displays concern and initiative;
  • Cooperates and maintains good rapport with staff, other departments, members, providers, and agencies involved in providing quality care to the member;
  • Maintains the standards of accurate and complete recording and reporting;
Minimum Requirements of the Position
  • High school graduate or equivalent.
  • One to three years of customer service experience. Prior experience in the insurance industry preferred.
  • Excellent communication and customer service skills.
  • Ability to learn and adapt;
  • Proficient in the use of Microsoft Office Suite.
