• MD/DO from an accredited educational institution
• ACGME Fellowship trained in Interventional Pain Management
• ABMS Board certification/eligibility in Pain Management (must obtain board certification within 1 year of employment)
• Unrestricted license to practice medicine in the State of Texas (or eligible for application)
• Favorable malpractice history
• Basic Life Support/Advanced Cardiac Life Support certification
• DEA license
• BC/BE (Required)
