A great SNF / LTC Facility is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment!

• Assess referred patients requiring rehabilitation services, providing direct patient care, assessing medical condition, functional capabilities, limitations, restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to patient capacity and tolerance under physician direction.
• Set realistic and achievable goals, document and record patient condition, and educate patients and families in appropriate occupational therapy methods.

• Graduated from an accredited school.
• Current state license as an Occupational Therapist / OT, or proactively in the process of application for licensure.
• Current CPR certification.
• Provide skilled occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of occupational therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
• Must meet 90% productivity.
