Occupational Therapist (OT) - SNF ​/ LTC - Contract

Job Summary

A contract Occupational Therapist/OT is needed for an SNF / LTC Facility. The role involves assessment, treatment planning, and direct patient care under physician direction.

• Assess referred patients requiring rehabilitation services, evaluating medical condition, functional capability, limitations, and potential for rehabilitation.
• Establish and administer a treatment program with goals tailored to the patient’s capacity and tolerance.
• Set realistic treatment goals, document and record patient condition.
• Educate patients and families about appropriate occupational therapy methods.
• Provide skilled occupational therapy interventions in accordance with physician orders.
• Plan, evaluate, and modify care to meet occupational therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
• Maintain 90% productivity.

• Graduated from an accredited occupational therapy program.
• Current state license as an Occupational Therapist/OT or actively pursuing licensure.
• Current CPR certification.
