Requirements:
- Current Nursing License in the practicing state
- Valid Driver's License
- Current CPR Certification
- Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
- Proficiency in electronic medical charting and time-keeping software

Roles & Responsibilities :
- Administer skilled nursing care to patients in their place of residence as prescribed by the physician.
- Assist in providing nursing services, identifying patients' needs, and reporting outcomes to the assigned RN Case Manager.
- Participate in planning and implementing care in conjunction with the RN to ensure quality patient care.
- Communicate effectively with patients/families, co-workers, and healthcare professionals, documenting care outcomes clearly.
- Support patients and families in learning self-care techniques and assess their responses to care.
- Assist in specialized duties related to end-of-life care for hospice patients as needed.
