Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
• Proficiency in electronic medical charting and time-keeping software

Roles & Responsibilities:

• Administer skilled nursing care to patients in their place of residence as prescribed by the physician.
• Assist in providing nursing services, identifying patients' needs, and reporting outcomes to the assigned RN Case Manager.
• Participate in planning and implementing care in conjunction with the RN to ensure quality patient care.
• Communicate effectively with patients/families, co-workers, and healthcare professionals, documenting care outcomes clearly.
• Support patients and families in learning self-care techniques and assess their responses to care.
• Assist in specialized duties related to end-of-life care for hospice patients as needed.
