• Conduct adult/geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
• Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities.
• Work assessments Sunday through Saturday from 8 am–7 pm, and be available during day, evening, and/or weekend based on your schedule.

• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• ~ 1 year of NP experience preferred, new graduates encouraged to apply~.
• Strong computer skills and familiarity with employee health/medical record software.
• Ability to travel to see members outside your home location.
• Willingness to pursue additional state licensure and credentialing as needed.

• Full‑time (FT) opportunities: PRN providers can transition to FT roles supporting their home location, a state, or region, and receive a FT sign‑on bonus.
