As an LVN on our skilled team, you’ll provide one-on-one care to patients in their homes
, helping them receive the care and support they need in a familiar environment. Your responsibilities may include:

• Providing skilled nursing care according to the patient’s plan of care
• Wound care
• Medication administration
• Monitoring and reporting changes in patient condition
• Communicating with the care team regarding patient needs and progress
• Building meaningful relationships with patients and their families

LVNs who enjoy independence, variety, and continuing to build their clinical skills tend to thrive on our skilled team.

• Active LVN license in Texas
• 1+ year of LVN experience
• Wound care experience preferred, but not required
