Registered Nurse; RN - Home Health

Job Overview

Graham Healthcare Group is seeking a Registered Nurse (RN) for a Home Health position in the Baden, PA area (Ambridge, Pennsylvania). This role is a weekend position with ongoing employment, 40 hours per week, 8‑hour shifts, and a $10,000 first‑year success bonus. The successful candidate will provide skilled nursing care to patients in their homes.

• Deliver skilled, patient‑centered care in a home health setting and collaborate with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently while never alone.
• Travel to patient homes within the assigned territory.

• Active Registered Nurse license in the state of employment.
• At least one year of RN experience and clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend approximately 20% of the day driving to/from patient locations.
• Successful completion of a drug screen as part of the background process.
