Physical Therapist – Full Time – Stonehenge American Fork, UT

Stonehenge American Fork is a post-acute care facility offering long‑term and short‑term care with a dedicated in‑house therapy team. We are seeking a full‑time Physical Therapist to join our rehabilitation program and provide high‑quality care to our geriatric clientele.

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, and develop appropriate care plans in line with regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Administer various procedures such as use of supportive and assistive devices as part of the rehabilitation plan.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries, ensuring compliance with regulatory and clinical requirements.

• Physical Therapy license required.
• Open to all experience levels, including new graduates.
