Occupational Therapist – Full Time, Stonehenge American Fork

Stonehenge American Fork in Utah is a post‑acute care facility offering long‑term, short‑term, and geriatric services. We provide a stable, in‑house therapy team that works with geriatric clients on their recovery journey.

• Screen and evaluate patients with cognitive and/or ADL/physical functioning disorders, developing an appropriate plan of care following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment interventions for patients with cognitive and/or ADL/physical functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures such as ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
• Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness, modify services based on evaluation, and refer as needed based on outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summaries of services provided per regulatory and clinical requirements.

• Occupational Therapy license required.
• Open to occupational therapists at all experience levels; new graduates are welcome.
