Experience:

1 to 2 years preferred.

Medical Social Worker Responsibilities:

Provide supportive counsel aligning to the patient’s and family needs

Assisting patients and their families to understand, accept and follow medical recommendations

Help utilize community resources available for the patient and their family

Maintains collaborative relationships with organization personnel to support patient care Assists physician and other team members with understanding significant social and emotional factors related to the patient’s health

Help integrate clinical guidelines, protocols and other metrics in development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of care Maintain clinical records on all patients referred to social work Other duties as delegated by the Clinical Director/Supervisor Job Type: Part-time to full time

Pay: $20.00 - $25.00 per hour Benefits:
