Dignity Home Health & Hospice On-Call Registered Nurse (RN)As our team continues to grow, we are looking for a reliable and compassionate On-Call Registered Nurse (RN) to provide support to patients and their families outside of normal business hours. This position is a great fit for an experienced nurse who is committed to patient-centered care, confident working independently, and values being part of a collaborative and supportive team.

Hours:
Friday at 5:00 p.m. through Monday at 7:55 a.m.
The On-Call RN provides clinical support, patient assessment, education, and hands-on care during scheduled evenings, nights, weekends, and holidays. As an after-hours resource, you will be an important point of contact for patients, families, caregivers, and facility staff when they need clinical assistance outside of regular office hours.
Essential Responsibilities- Address patient needs after hours through telephone triage and in-person visits when necessary.
- Perform comprehensive patient assessments in a variety of settings, including private residences, assisted living communities, and skilled nursing facilities.
- Provide knowledgeable guidance, support, and reassurance to patients, families, caregivers, and facility personnel.
- Conduct after-hours admissions when needed.
- Complete thorough, accurate, and timely clinical documentation.
- Maintain open communication with the interdisciplinary care team to promote coordinated and consistent patient care.
- 1 year of home health and/or hospice nursing experience preferred.
- Strong clinical assessment abilities and sound critical-thinking skills.
- Dependable, composed, and professional when responding to urgent or unfamiliar situations.
- Compassionate and respectful approach to patient and family care.
- Highly organized with strong attention to detail and follow-through.
- Excellent verbal and written communication skills.
- Comfortable working with electronic medical records and other healthcare technology.
- Dependable transportation for travel throughout the service area.
- Current Utah Registered Nurse (RN) license — required
- Current CPR certification — required
- Valid Utah driver’s license and current automobile insurance — required
