Gentiva Hospice seeks a Nurse Practitioner to deliver advanced clinical care across hospice and palliative settings. You will perform patient management visits, hospice Face-to-Face (F2F) encounters for eligibility certification, and partner with IDG/IDT to ensure compliant, compassionate care.
You will manage complex symptoms, contribute to care planning with families, and support transitions across home, SNF, and assisted living settings. Strong NP credentials and hospice experience preferred.
