Position:
Social Services Tech 1 – Sumter County Health Department (grant‑funded position until March 2029).
Location:
Sumter County Health Department, Georgia. The position is not remote.
- Engage in recruitment activities as applicable.
- Complete initial and annual family‑centered assessments.
- Develop, monitor, and review goals with each family.
- Provide group sessions and one‑to‑one home visits focused on health and development, planning, delivering, and documenting each visit.
- Travel to conduct home visits.
- Administer appropriate screenings to expectant and postpartum mothers, children, and families.
- Provide group connections to build social connections, engage in parent‑child interaction activities, and increase knowledge of child development support.
- Connect families to resources and help transition to other services as needed.
- Maintain and submit all required family and program documentation promptly.
- Participate in at least 2 hours of reflective supervision monthly.
- Participate in at least 2 hours of staff meetings monthly.
- Obtain competency‑based professional development.
- Participate in continuous quality improvement.
- High School Diploma or GED.
- One year of experience in a social‑services related position.
- Bachelor’s Degree in a social‑services related field from an accredited institution.
- One year of experience in a social‑services related field.
- Valid driver's license.
