As a Case Manager, you will oversee and coordinate all aspects of patient care, develop individualized care plans, and collaborate with an interdisciplinary team to ensure the highest standards of health and wellness.

Responsibilities
  • Patient Assessment & Care Planning:
    Conduct initial and ongoing assessments of patients and their families to determine home care needs. Develop and update individualized care plans based on health assessments, including therapeutic, preventive, and rehabilitative actions.
  • Direct Patient Care:
    Administer medications and treatments as prescribed, provide skilled nursing care per the State Nurse Practice Act, and implement preventive and rehabilitative procedures. Offer guidance to patients and families to help them understand and manage their health.
  • Care Coordination:
    Coordinate care across services and ensure seamless support. Collaborate with physicians and community health providers to optimize care plans and outcomes.
  • Documentation & Communication:
    Maintain accurate and timely clinical notes, updating the primary physician at least every 60 days or as needed. Communicate any changes in patient status to the physician and obtain necessary orders.
  • Patient & Family

    Education:

    Provide health instructions tailored to the patient’s needs and care plan. Include families in the planning process to ensure they are well-informed and supported.
  • Supervision of Support Staff:
    Instruct, supervise, and evaluate home health aides at least bi-weekly to ensure quality care delivery.
Qualifications
  • Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.
  • Current RN licensure and CPR certification.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
