Registered Nurse (RN) - Home Health / Hospice

Position Overview

We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality care to patients in their homes. This position is ideal for an RN who enjoys working independently, building relationships with patients and families, and coordinating care as part of an interdisciplinary healthcare team.

The RN will complete Medicare Home Health visits
, perform patient assessments, coordinate plans of care, provide education and symptom management, and maintain accurate clinical documentation.

Homecare Homebase (HCHB) experience is preferred.

What You'll Do

Complete approximately 6-8 patient visits per day

Perform initial and ongoing nursing assessments

Provide skilled nursing care in patients' homes

Develop and implement individualized Plans of Care

Monitor patients for changes in condition and communicate concerns to physicians

Provide patient and caregiver education, including medication management, dressing changes, oxygen use, and symptom management

Coordinate care with physicians, LPN/LVNs, CNAs, and other interdisciplinary team members

Provide support and education to patients and their families

Maintain timely and accurate clinical documentation

Follow Medicare, Medicaid, infection-control, and regulatory guidelines

Participate in staff meetings and quality improvement activities

Participate in the agency's on-call rotation when required

Qualifications

Required:

Active and unrestricted Registered Nurse (RN) license in the state of employment

Minimum 1 year of RN experience in acute care, oncology, long-term care, hospice, or home health

Current American Heart Association BLS

Valid driver's license

Reliable transportation and willingness to travel within a 60-mile radius

Strong patient assessment, documentation, communication, and care-coordination skills

Preferred:

Knowledge of Medicare/Medicaid guidelines

Experience caring for patients with complex or end-of-life needs
