Position: RN/LPN 3p -11p Full Time

Overview

River Woods is currently seeking a Full Time (32 hours/week) RN or LPN to provide direct resident care in a team-focused environment where our residents are at the heart of what we do.

$3000 Sign on Bonus

Responsibilities
  • Perform professional duties in accordance with facility policies, New Hampshire Nurse Practice Act, OBRA and HIPAA
  • Collaborate with interdisciplinary team members to formulate and update individualized resident care plans, participating in Resident Care Plan Meetings
  • Implement care protocols including falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control and pain assessment and management
  • Assess and document changes in residents’ physical or mental status and report findings to the primary care physician and treatment team
  • Provide leadership and direction for direct caregivers, preparing daily assignments and supervising direct care through observation and evaluation
  • Establish positive working relationships with team members, residents and family members to facilitate open, direct communication
  • Obtain, administer, and document medications and treatments in accordance with professional guidelines and nursing department policies, inventory and maintain locked system for narcotics
  • Support, create, and maintain a positive culture for team members and residents
Qualifications
  • Minimum of a Nursing Degree or Diploma from an accredited college, university or nursing school
  • Long-term care facility experience and specialized knowledge of elderly and rehabilitative nursing is preferred
  • Registered nursing (RN, LPN) license within the State of New Hampshire; intravenous administration certification preferred; must maintain CEUs annually; CPR required
