Overview
River Woods is currently seeking a Full Time (32 hours/week) RN or LPN to provide direct resident care in a team-focused environment where our residents are at the heart of what we do.
$3000 Sign on Bonus

- Perform professional duties in accordance with facility policies, New Hampshire Nurse Practice Act, OBRA and HIPAA
- Collaborate with interdisciplinary team members to formulate and update individualized resident care plans, participating in Resident Care Plan Meetings
- Implement care protocols including falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control and pain assessment and management
- Assess and document changes in residents’ physical or mental status and report findings to the primary care physician and treatment team
- Provide leadership and direction for direct caregivers, preparing daily assignments and supervising direct care through observation and evaluation
- Establish positive working relationships with team members, residents and family members to facilitate open, direct communication
- Obtain, administer, and document medications and treatments in accordance with professional guidelines and nursing department policies, inventory and maintain locked system for narcotics
- Support, create, and maintain a positive culture for team members and residents
- Minimum of a Nursing Degree or Diploma from an accredited college, university or nursing school
- Long-term care facility experience and specialized knowledge of elderly and rehabilitative nursing is preferred
- Registered nursing (RN, LPN) license within the State of New Hampshire; intravenous administration certification preferred; must maintain CEUs annually; CPR required
