Licensed Practical Nurse

River Woods Exeter is seeking compassionate RN/LPN nurses to provide high-quality care to older adults in our continuing care retirement community. In this role, you will deliver hands-on clinical care, administer medications, monitor resident conditions, and collaborate with an interdisciplinary team to support resident well-being across independent, assisted living, and nursing care settings. Responsibilities

• Provide direct nursing care to residents across independent, assisted living, and nursing care levels
• Administer medications and treatments per provider orders and facility protocols
• Monitor resident conditions, perform assessments, and document accurately in medical records
• Collaborate with interdisciplinary team to develop and update care plans
• Educate residents and families on health conditions, medications, and wellness practices
• Support a compassionate, resident-centered environment that promotes dignity and respect
• Respond to emergencies, perform interventions, and follow safety and infection control procedures
• Supervise and support CNAs and other care staff to ensure quality standards
• Participate in quality improvement, care conferences, and regulatory compliance activities
• Communicate changes in resident status promptly to providers and families
