Registered Nurse - LTC​/SNF

Job Description

Job Summary:

Seeking a Skilled Nursing Facility (SNF) RN with at least 2 years of experience to provide quality patient care in a long-term care setting. Must hold a current NYS license and BLS certification.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor patient conditions and document clinical observations thoroughly.
• Collaborate with multidisciplinary teams to develop and update care plans.
• Ensure compliance with state regulations and facility policies.
• Provide compassionate bedside care to promote patient well-being.

Qualifications:

• Valid NYS RN license required.
• Minimum of 2 years' experience in a SNF or LTC setting.
• Current BLS certification.
• Strong clinical assessment and documentation skills.
• Ability to work effectively independently and as part of a team.
