Position: LVN / LPN - LTC/SNF
Job Description

Job Summary:

Provide skilled nursing care to residents in a Skilled Nursing Facility as an LPN. Ensure optimal patient outcomes through efficient clinical support and adherence to licensing standards.

Responsibilities:

• Administer medications and treatments accurately according to physician's orders.
• Monitor residents' health status and report changes promptly.
• Document clinical procedures and patient responses thoroughly.
• Collaborate with healthcare team to develop and implement care plans.
• Adhere to facility policies, safety protocols, and state licensing requirements.

Qualifications:

• Hold a valid NYS LPN license with a minimum of 2 years of experience in a SNF setting.
• Maintain current BLS certification.
• Demonstrate strong clinical skills and attention to detail.
• Possess excellent communication and teamwork abilities.
• Be willing to work within a local travel radius of 50 miles (for travelers).
